Upload your prescription records, physician letters, or pharmacy dispensing records securely for Medical Review Officer (MRO) review. Documents are accessible only to authorized ScreenClear staff and the MRO — collectors never receive them.

Your Information

Please provide your identifying information exactly as it appears on your test requisition form.

You must be at least 18 years old to submit documentation.

Your Documents

PDF, JPG or PNG; up to 5 files, 10 MB each. You may also submit without documents and provide them to the MRO directly.

Your Privacy & Consent (PIPEDA)

By submitting, you consent to ScreenClear collecting the information and documents you provide, for the sole purpose of Medical Review Officer review of your drug test result:

  • Your documents are accessible only to authorized ScreenClear staff and the Medical Review Officer. Collectors never receive them.
  • Every access to your documents is recorded in an audit log.
  • In accordance with PIPEDA, your documents are retained for two years from submission and then permanently deleted — unless a longer retention period is required, for example by law.
  • You may request access to your information, or its deletion, at any time by contacting [email protected].
