Collector Concern or Incident

Use this pathway for a collector service concern, collection incident, documentation issue, or pharmacy-site concern. It is not a substitute for immediate collection controls, the Day-of-Collection Checklist, required specimen handling, or urgent escalation.

When to report

Report factual concerns involving a collection process, donor interaction, identity or document issue, CCF completion concern, packaging or seal issue, device or kit concern, pharmacy-site issue, service experience, or another matter that may require ScreenClear review.

Protect the collection first

Follow the applicable collection procedure before reporting. Do not alter, discard, conceal, or replace a CCF, specimen, seal, device, record, or original document. Keep required documents and any relevant original materials together under the normal ScreenClear document-handling process.

Information to record

Provide the collection date and time, assigned pharmacy or service location, applicable donor, service, CCF, or kit reference, a factual description of what occurred, immediate actions taken, and any relevant people or records. State only what you observed or did; do not include opinions, diagnoses, or presumptive test-result language.

Supporting information

This secure form supports controlled uploads of relevant documents or photographs. Use only materials needed for the review. Continue to retain required originals in the ScreenClear document folder and follow existing upload routing for materials that are already captured through the day-of workflow.

ScreenClear review

An authorized reviewer will assess the report, obtain clarification if needed, and document the outcome. Public case tracking will never display internal notes, donor information, pharmacy-site details, or restricted investigation information.
